NCCARE360 is available in all 100 North Carolina counties and helps electronically connect individuals with needed support. 

Many North Carolinians struggle with food insecurity, housing instability, lack of access to transportation, and other health-related needs every day. NCCARE360, the first statewide coordinated care network, better connects individuals with local services and resources.

The network bridges gaps in a fragmented health and human services system by connecting providers and organizations across sectors through a shared technology platform.

Providers can connect individuals and families who have unmet social needs with community resources electronically. The platform enables easy feedback and follow-up to help close the care loop for individuals and families seeking help.

Healthy Opportunities Pilot: a first-of-its-kind program

Powered by the NCCARE360 platform, the Healthy Opportunities Pilot (HOP) is the nation’s first comprehensive program to test and evaluate the impact of providing nonmedical services—like housing, food, transportation, interpersonal safety, and stress management—to people with Medicaid insurance. HOP services are available in select counties across North Carolina, and the NC Department of Health and Human Services (NCDHHS) administers the program.

Medicaid coverage has expanded in North Carolina!

More people now qualify for NC Medicaid, including those between 19 and 64 years old with higher incomes. Even if you didn’t qualify before, you might be eligible today. NC Medicaid covers doctor visits, annual check-ups, emergency care, dental care, mental health services, and more—at little or no cost to you. Learn more at Medicaid.nc.gov.
