NCCARE360 is the first statewide coordinated care network that better connects individuals to local services and resources. Many North Carolinians struggle every day with food insecurity, housing instability, lack of transportation access, or other social needs. NCCARE360 provides a solution to a fragmented health and human services system by connecting providers and organizations across sectors in a shared technology network. In the NCCARE360 network, providers and organizations can electronically connect individuals and families who have unmet social needs to community resources. NCCARE360 also allows for easy feedback and follow-up so that individuals and families seeking help are less likely to fall through the cracks.

Navigating services like health, housing, employment, and transportation can be incredibly difficult, but these factors all contribute to our overall health. NCCARE360 provides the opportunity for health to all North Carolinians by providing public access to resources and helping health and community-based organizations make electronic referrals, communicate in real time, securely share client information, and track outcomes together.
