About NCCARE360

NCCARE360 is the first statewide coordinated network that unites health care and human services organizations with a shared technology platform allowing for a coordinated, community-oriented, person-centered approach for delivering care in North Carolina. NCCARE360 allows providers to electronically connect those with identified needs to community resources and allows for a feedback loop. This solution ensures accountability around services delivered, provides a “no wrong door” approach, closes the loop on every referral made, and reports on the outcomes of that connection. NCCARE360 will be available in all 100 counties by the end of 2020, statewide in North Carolina. 

NCCARE360 has multiple functionalities including:

  • A robust statewide resource directory that will include a call center with dedicated navigators, a data team verifying resources and text and chat capabilities.

  • A community resource repository to integrate multiple resource directories across the state to share data.

  • A shared technology platform that enables health and human services providers to send and receive electronic referrals, seamlessly communicate in real-time, securely share client information, and track outcomes.

  • A community engagement team working with community-based organizations, social service agencies, health systems, independent providers, community members and more to create a statewide coordinated care network.

    While this technology is robust and transformative, technology is just technology unless all network partners are trained and empowered to use it to better serve their patients and clients. NCCARE360 has a community engagement team that directly supports the community through every step of joining the network to ensure each partner is supported. As new regions launches, the team remains engaged in the community, working with partners to regularly review data and network performance, solicit feedback and input on processes, and provide on-going technical assistance

A Partnership Dedicated to Improving Health

NCCARE360 is the result of a strong public-private partnership between the NC Department of Health and Human Services (DHHS) and the Foundation for Health Leadership & Innovation (FHLI). NCCARE360 is guided by FHLI’s more than 35 years of experience in successfully developing and supporting innovative programs and building and advancing partnerships that improve health. NCCARE360 is an important component in DHHS’ Healthy Opportunities strategy and integral to DHHS’s mission is to improve the health, safety, and well-being of all North Carolinians.
The NCCARE360 implementation team includes United Way of NC/2-1-1, Expound Decision Systems, and Unite Us. United Way of NC/2-1-1 has more than a decade of experience connecting individuals and families across North Carolina to free and confidential information on local health and human services resources, through a simple dialing code 2-1-1, and a statewide resource database and call-center. Expound Decision Systems has created a successful data repository model which will be extended to manage resources specific to the Social Determinants of Health in North Carolina. Unite Us provides the care coordination and outcomes platform for NCCARE360 that allows healthcare providers, community-based organizations, and others to work together securely in real-time.
