Building Connections for a

Healthier North Carolina

Many North Carolinians struggle every day with food insecurity, housing instability, lack of transportation access, or other non-medical social needs. NCCARE360 is the first statewide coordinated care network that better connects individuals to local services and resources. NCCARE360 provides a solution to a fragmented health and human services system by connecting providers and organizations across sectors in a shared technology network. In the NCCARE360 network, providers can electronically connect individuals and families who have unmet social needs to community resources. NCCARE360 also allows for easy feedback and follow-up to help close the care loop for individuals and families seeking help.

NCCARE360 is available in all 100 counties of North Carolina and helps electronically connect any individual to needed support. The Healthy Opportunities Pilot (HOP) is the nation’s first comprehensive program to test and evaluate the impact of providing select evidence-based, non-medical interventions related to housing, food, transportation and interpersonal safety and toxic stress to high-needs Medicaid enrollees in select counties across the state. This program is administered by the NC Department of Health and Human Services. If you are looking for more information on HOP, please visit the NCDHHS website.

North Carolina has expanded health care coverage to more people. NC Medicaid now covers people ages 19 through 64 years with higher incomes. You may be able to get health care coverage through Medicaid even if you did not qualify before. NC Medicaid pays for doctor visits, yearly check-ups, emergency care, dental care, mental health and more – at little or no cost to you. To learn more, go to Medicaid.nc.gov.

Together, we are building a healthier North Carolina.

Navigating services like health, housing, employment, and transportation can be incredibly difficult, but these factors all contribute to our overall health. NCCARE360 provides the opportunity for health to all North Carolinians by providing public access to resources and helping health and community-based organizations make electronic referrals, communicate in real time, securely share client information, and track outcomes together.

“Through NCCARE360 we connected [a lady whose home was about to be condemned] to another agency and her issue was tracked and resolved within 24 hours. NCCARE360 eliminates unnecessary barriers for the client and the provider. Now the provider can see the services the client was receiving as they deliver care for the same person.”

Jackie Lucas
Executive Director, Salvation Army of Greensboro Center of Hope

“We made a referral at 10 p.m. and it was accepted at 8 a.m. the next morning. I was so excited to tell our client that she would get the help she needed. I’ve been sharing what NCCARE360 is with everyone because this is an amazing resource. This is really going to help so many people. I hope we get more community members to get onboard because I think it’s phenomenal.”

Tiffany Joseph
