Interested in joining NCCARE360?
NCCARE360 connects community-based organizations, health plans, and health systems across the state. If you are an organization administrator, supervisor, or decision-maker, fill out the inquiry form for more information about joining the NCCARE360 network. If your organization currently utilizes NCCARE360 and you want to be an authorized user, please get in touch with your supervisor to ask about gaining access.
Want to learn more before you join? Filling out the inquiry form does not commit you to joining the platform, but it will enable you to learn more and ask questions to ensure NCCARE360 is the right solution for you and your community. You can also join NCCARE360 at one of our monthly information sessions, held every third Friday of the month from 12 to 1 p.m.
