NCCARE360 supports coordinated, community-oriented, and person-centered care in all 100 North Carolina counties. By enabling feedback and follow-up, the platform creates a “no wrong door” approach for providers to connect people with the community resources they need.

Resource Navigators

Resource Navigators address complex NCCARE360 referrals by supporting individuals who request assistance and community-based organizations unable to follow clients through the referral process.

Resource Directory

A dedicated NC 211 resource team maintains and updates our resource directory. It features a robust database of programs and services integrated into the NCCARE360 platform in real time.

Technology Platform

Unite Us powers the NCCARE360 technology platform. It enables providers and social service organizations to collaborate and serve clients by identifying unmet social needs, sending and receiving secure electronic referrals, and tracking outcomes.

Training & Support

Unite Us provides comprehensive training and technical support for all platform users. During and after onboarding, the Unite Us team can answer questions and troubleshoot technical issues to empower everyone in the network to serve their patients and clients better.

Meet the NCCARE360 Team

Administrators

The Foundation for Health Leadership & Innovation (FHLI) guides the NCCARE360 program, drawing on 40+ years of experience successfully developing and supporting innovative, community-driven programs and partnerships that build a healthier North Carolina. With a long history in the state and a deep understanding of its health and wellness needs, FHLI scales solutions that benefit every resident. The organization serves as the NCCARE360 administrator, ensuring the program runs smoothly.

Full Bio

Nicole leads the daily operations of NCCARE360, fostering collaboration among cross-sector partners and ensuring program deliverables, timelines, and communications are effectively managed.

With nearly two decades of experience in program and project management, communications, and client support for health-focused organizations in North Carolina and abroad, Nicole is dedicated to advancing health equity by addressing social determinants of health and implementing community-driven solutions. Prior to joining FHLI, she served as Program Coordinator for the Social & Behavioral Sciences Core at Duke University’s Center for AIDS Research (CFAR).

Nicole holds a Master of Public Health with a concentration in health behavior, a Bachelor of Arts in Psychology and Spanish from East Carolina University, and the Project Management Professional (PMP) certification from the Project Management Institute.

Full Bio

Amari Bass is a dedicated professional with a passion for health equity, community health, supporting underserved populations, and enhancing healthcare operations. Before joining FHLI, Amari worked as a Community Health Worker, serving as a bridge between the Wake County community and its resources, with a strong commitment to improving quality of life.

In addition to this role, Amari is actively involved in professional organizations, serving as a member of the North Carolina Chapter of the National Association of Health Services Executives (NAHSE).

Amari holds a Bachelor of Science in Public Health with a Community Health concentration from East Carolina University and a Master of Healthcare Administration from Winston Salem State University and is a Certified Health Education Specialist.

Implementation Team

United Way of NC/NC 211

United Way of NC (UWNC)/NC 211 has over a decade of experience connecting individuals and families across North Carolina with verified local health and human services resources through a simple dialing code (2-1-1) and website (nc211.org). NC 211 is free, confidential, and available 24/7/365 in most languages.

UWNC and NC 211 directly support NCCARE360’s robust statewide directory, verifying and updating 13,000+ programs and services. Based in NC 211 call centers, NCCARE360 navigators can provide timely support for complex referrals and assistance requests.

Unite Us

Unite Us powers the shared technology platform that enables health and human services providers to send and receive electronic referrals, communicate in real-time, share client information securely, and track outcomes. In North Carolina, Unite Us supports users in the NCCARE360 network.

These providers include those who work at case management agencies, shelters and housing assistance organizations, medical and mental health clinics, schools, older adult support service agencies, transportation services, food pantries, churches, colleges, veterans groups, and parks and recreation departments.

North Carolina Department of Health and Human Services

The North Carolina Department of Health and Human Services (NCDHHS) is a formative partner in creating and implementing NCCARE360. As a keystone in the state’s nation-leading approach to whole-person care, NCCARE360 is an integral component of NCDHHS’s mission to improve health, safety, and well-being for every North Carolinian.

The NCCARE360 platform is available free of charge to a diverse network of nonprofits, schools, early childhood centers, and government agencies. It facilitates payments through Healthy Opportunities Pilots and helps clinicians using the NCDHHS Standardized Social Determinants of Health Screening Questions connect patients with community resources.
