NCServes Joins Forces with NCCARE360, Bringing Coordinated Care and  Resources to North Carolina’s Military Community

RALEIGH, NORTH CAROLINA – NCCARE360, the nation’s first statewide coordinated  care network connecting individuals to local services and resources, announced today that NCServes, North Carolina’s technology network of public, private and non-profit  organizations serving military families, is merging its network of resources with  NCCARE360’s this summer.

“Unifying the technology and resources of NCCARE360 and NCServes brings an  improved experience for North Carolina’s military community,” said Jeff Smith, Military and Veterans Program Liaison, North Carolina Department of Health and Human Services. “By joining forces, we will increase the number of resources available to our  active-duty service members, veterans and their families.”

Launched in 2015, NCServes is the nation’s first statewide coordinated network of  public, private, and non-profit organizations working together to connect military service  members, veterans, and military families with providers and resources for which they are eligible. All NCServes network partners will now have access to additional NCCARE360 features and data to help them better serve military families. Implementation is currently underway, and the combined network launch is expected in mid-July of this year.
“Joining these two robust resource platforms will not only provide improved access to  services and resources for our state’s military families,” said Kelly Calabria, President  and CEO of FHLI. “But the partnership builds on NCCARE360’s mission to better address the health needs of all North Carolinians.”

NCCARE360 Brings State’s Seven Local Management Entities/Managed Care Organizations into Its Network to Coordinate Whole-Person Health

APRIL 13, RALEIGH, NORTH CAROLINA – NCCARE360, the nation’s first statewide coordinated care network connecting individuals to local services and resources, announced today that all seven of North Carolina’s behavioral health managed care organizations have joined the NCCARE360 network and will use the platform to coordinate whole-person care, promote health equity, and address the social drivers that influence their beneficiaries’ health. 

The seven organizations, called Local Management Entities/Managed Care Organizations or LME/MCOs, are responsible for managing and monitoring the provision of public mental health, intellectual and developmental disabilities, and substance use disorder services for people who are uninsured or are covered by Medicaid. LME/MCOs will use NCCARE360 in the regions they serve to better connect people across North Carolina who need critical services including food, housing, transportation, mental health services, and employment.

“LME/MCOs have deep experience in serving populations with significant behavioral health needs, substance use disorders, and intellectual and developmental disabilities,” said Dave Richard, Deputy Secretary of NC Medicaid. “LME/MCOs will use NCCARE360 as the infrastructure to strengthen connectivity between care providers, case management, and community-based organizations in an effort to reduce barriers, promote equity, and deliver better health for the people they serve and their communities.”

NCCARE360 will allow LME/MCOs to make electronic referrals, communicate in real time, securely share client information, and track outcomes through the Unite Us Platform. Community members can search NCCARE360’s repository of more than 10,000 local services at nccare360.org/resources or access resources by contacting NCCARE360 Navigators through nccare360.org/request-assistance.

“We are excited to welcome the LME/MCOs to the platform and amplify their impact by quickly connecting people to multiple providers across all medical and non-medical service types and allowing them to seamlessly coordinate care across a community-wide care team,” said Danika Mills, North Carolina State Director, Unite Us.

North Carolina’s LME/MCOs recently completed a Request for Application to become Behavioral Health and I/DD Tailored Plans under North Carolina’s continued path towards Medicaid Managed Care transformation. Under Medicaid transformation, Tailored Plans will provide physical health, behavioral health, long-term care, pharmacy services and unmet health-related resource needs to qualifying North Carolinians under one plan. These plans will provide the same services as Standard Plans but will also provide additional, specialized services that serve individuals with significant behavioral health conditions, I/DDs and traumatic brain injuries. 

“As the state moves closer to launching Medicaid managed care, bringing the LME/MCOs onto the NCCARE360 platform is an exciting step,” said Kelly Calabria, CEO and President of the Foundation for Health Leadership & Innovation, which oversees and administers NCCARE360. “The purpose of NCCARE360 is to improve the health and well-being of all North Carolinians by recognizing there are many aspects that affect our health. Since the development of NCCARE360’s statewide network, health and human service providers in all 100 North Carolina counties have made more than 100,000 electronic referrals to connect individuals and families to the resources they need to stay healthy and well.

North Carolina Creates Nation’s First Statewide Infrastructure Connecting Healthcare and Human Services

NCCARE360 Now in All 100 Counties to Help North Carolina Respond to and Recover from COVID-19

June 22, 2020 – Through a public-private partnership, North Carolina has created the nation’s first statewide technology platform, NCCARE360, to coordinate whole-person care uniting traditional healthcare settings and organizations that address non-medical drivers of health, such as food, housing, transportation, employment and interpersonal safety. NCCARE360 is now available in all 100 counties.

“All kinds of factors can affect people’s health, and our groundbreaking and innovative NCCARE360 brings government together with the private sector to help people deal with the all the challenges of being sick with COVID-19 or any other health problem,” said Governor Roy Cooper.

Now, anyone in North Carolina can request services and be connected to an accountable organization to support their immediate and long-term needs. NCCARE360 enables health and community-based organizations to make electronic referrals, communicate in real time, securely share client information, and track outcomes together. North Carolinians who need assistance can search NCCARE360’s repository of more than 10,000 local services at nccare360.org/resources, access resources by calling 2-1-1, or complete an online form at nccare360.org/request-assistance.

“Having this critical infrastructure in place puts our state in a stronger position to address the devastating impact COVID-19 is having on so many of our residents and to help us recover. NCCARE360 completed its statewide rollout six months ahead of schedule as the team fast-tracked the statewide expansion in response to COVID-19. Since the network launched in 2019, more than 1,000 organizations across the state have joined the network to connect the people they serve to the resources they need to stay healthy and well, while also ensuring accountability and tracking outcomes

“NCCARE360 is a model for the nation. It breaks down silos that create barriers to needed care and services – particularly in rural communities – and exacerbate health disparities. It was critical before COVID-19, and it is even more so now,” said Georgina Dukes, Unite Us Network Director.

Research has established that having an unmet resource need — including experiencing food insecurity, housing instability, unmet transportation needs and interpersonal violence or toxic stress — can negatively impact health while also increasing health care utilization and costs. These non-medical drivers have the greatest impact on health, yet historically the human services and healthcare systems have been fragmented and siloed. NCCARE360 breaks down those siloes, putting people at the center of service delivery.

“In order to improve the health of individuals we must have the ability to understand their unique needs and connect them to resources in their local community. NCCARE360 Steps Up COVID-19 Response, Onboards 19 More NC Counties and a Major Health System

May 4, 2020 – NCCARE360, the statewide coordinated care network connecting individuals to local services and resources, has expedited its expansion efforts in response to the COVID-19 health crisis, launching the network in 19 additional North Carolina counties while also extending its reach into eastern North Carolina with Vidant Health as the most recent health system to join the network.

“To be responsive to the growing needs of people during the pandemic, NCCARE360 is moving quickly to onboard health care and human service organizations in the remaining counties that have not yet been a part of the coordinated network,” said Dr. Elizabeth Cuervo Tilson, who serves North Carolina as the State Health Director and the Chief Medical Officer for the Department of Health and Human Services. “By being flexible and offering statewide virtual meetings, rapid response registration, and online software training, we have helped our communities connect and have ensured that people have access to the vital resources they need.”

The most recent counties to join NCCARE360 are Avery, Burke, Caldwell, Caswell, Cabarrus, Duplin, Greene, Harnett, Jones, Lee, Lenoir, McDowell, Mitchell, Onslow, Polk, Rutherford, Surry, Wayne, and Yancey counties. This latest regional rollout has launched with health care and community-based organizations, including the Salvation Army of Lee County, Wayne Action Group for Economic Solvency, Inc, Duplin County Partnership for Children, and Feeding Avery Families among others. More than 1,000 organizations across the state have joined the network and a current status can be found at NCCARE360.org.

NCCARE360 is a collaborative solution to health care and service delivery systems that, in the past, have been fragmented and difficult to navigate. NCCARE360 seamlessly connects North Carolinians to behavioral health, clinical, and social service partners who are dedicated to working together to improve health and well-being.

The integration between Vidant Health’s electronic health record, Epic, and the Unite Us technology is the third successful partnership for NCCARE360 and a major North Carolina health system. This integration permits single sign-on for healthcare users working directly with patients who are need of social services. Clinicians, case managers, and additional hospital staff are now able to connect patients directly with community resources without leaving their electronic health record.

NCCARE360 Now Live in 50 North Carolina Counties

January 2, 2020 – NCCARE360, the statewide coordinated care network connecting individuals to local services and resources, has officially launched in 29 additional North Carolina counties. This sixth regional rollout has launched with health care and community-based organizations, including the Chatham Health Alliance, Halifax County Health Department, MANNA FoodBank, United Way of Davidson County, and Mountain Projects Community Action Agency.

Now serving a total of 50 North Carolina counties, NCCARE360 is on target to connect health and human service resources in every county and reach full statewide implementation by the end of 2020.

NCCARE360 Announces 1,000th Referral

RALEIGH — NCCARE360, the state’s first coordinated care network that knits together health care, human services and community-based organizations to deliver person-centered care, is proud to announce it has reached its 1,000th referral.

“A person’s needs are often interconnected, but our health care and social service systems have been fragmented and siloed,” said Dr. Elizabeth Tilson, State Health Director and Chief Medical Officer of NCDHHS. “The NCCARE360 coordinated network allows our communities to be connected in a system of health that supports our vision that all North Carolinians have the opportunity for health.”

One of NCCARE360’s first referrals was in May 2019 when a resident was able to find employment assistance through the network. A local church in the Triangle region submitted an employment assistance referral through NCCARE360 and, within hours, the resident was contacted by an employment agency. Most recently for NCCARE360’s 1000th referral, a Piedmont resident was referred to social services case management by a local health department’s WIC Nutrition services. As a result of the referral, the resident received diapers, bedding, and other necessities for the resident’s home.

“We are so proud that NCCARE360 is succeeding in the mission we’re hoping to accomplish,” said Anne Thomas, President and CEO of the Foundation for Health Leadership & Innovation, “which is providing a system that allows North Carolinians to easily and efficiently access the community services they need, in real time, through one integrated network.”

Since April, NCCARE360 has enabled health care providers, local health departments, social services agencies, community providers, and community-based organizations to make 1,000 electronic referrals connecting people with the resources they need. In addition to health care services from doctors and other providers, network providers can use NCCARE360’s shared technology to assist with identified needs such as housing, employment, food, interpersonal safety, and transportation, among others. The system allows for a feedback loop on the outcome of assistance, ensures accountability for services delivered, and reports the outcomes of these person-to-person connections.

To date, the NCCARE360 referral network has launched in 21 counties, including Alamance, Beaufort, Bertie, Brunswick, Chowan, Durham, Edgecombe, Franklin, Guilford, Granville, Hertford, Johnston, Martin, New Hanover, Pender, Person, Pitt, Rockingham, Vance, Wake and Warren. NCCARE360 will be available in all 100 counties by the end of 2020.

October 10, 2019 – NCCARE360, the statewide coordinated care network connecting individuals to local services and resources, has officially launched in Durham, Franklin, Granville, Person, Vance, and Warren Counties. This fifth regional rollout has launched with 94 new healthcare and community-based organizations, including Project Access, Durham County Public Health Department, ReBuild Communities, Franklin-Vance-Warren Opportunity, Inc., Centre for Homeownership and Economic Development, and El Futuro, Inc.

With growing awareness of the many social, economic, and environmental factors that influence health, it is critical to integrate services along the continuum. However, the current system to navigate these services is fragmented. When providers make referrals to other organizations, the process is often manual, time-consuming, and does not allow them to understand what happened to their client once the referral was sent.

NCCARE360 serves as a collaborative solution to this problem by providing a coordinated, community-oriented, person-centered approach to delivering care in North Carolina.

“NCCARE360 seamlessly pulls together referral partners and a community that cares. We are thrilled to participate in Durham’s launch linking traditional and non-traditional service providers to unlock the power referral-tracking to address our local healthcare needs,” shared Erika Cerwin, founder and CEO of BeSpree.

Trailblazing in North Carolina: A 6 Month Reflection

Greetings!

My name is Jimmy Fisher and I am the State Director, NCCARE360/Unite Us. Together, our team has been working hand in hand with communities across the state to help empower their efforts to provide individuals with more opportunities to achieve a greater level of health.  We believe in NCCARE360 and know it will truly transform North Carolina!

When our team began working in Guilford, Alamance, and Rockingham in January 2019, we couldn’t have predicted how truly amazing these communities are. Fast forward six months and they have helped us build a well-established coordinated network with strong partnerships across various sectors, truly weaving together services.

It takes an immense amount of fortitude and bravery to be trailblazers, and the people of Guilford, Alamance, and Rockingham counties have led the way for the state of North Carolina as the first regions to launch with NCCARE360.

When you are surrounded by people that lean on each other and keep an individual at the center of care, amazing things can happen. Through NCCARE360, we have seen individuals facing difficult situations receive support within hours. We were able to successfully integrate NCCARE360 into Cone Health’s Electronic Health Record (EHR) system, EPIC. The integration provides Cone Health clinical staff with a seamless way to make referrals and receive closed loop information directly to and from their EHR. The NCCARE360 network in these three counties has grown with new partners joining each week, and we are excited to see expansion in the coming years.

In June, we hosted our launch party for Guilford, Alamance and Rockingham counties and it was a great celebration! In those counties, the network has grown from 60 organizations to over 120 in very little time. Statewide, we have now onboarded over 300 organizations, connecting more individuals to resources they need to live healthier. Our team is so excited about the organic connections at the local level, as well as weaving together historically siloed sectors.

Collaborations Continue as NCCARE360 Reaches New Hanover, Brunswick, and Pender

August 28, 2019 – NCCARE360, the statewide coordinated care network connecting individuals to local services and resources, has officially launched in New Hanover, Brunswick, and Pender counties. This fourth rollout has launched with healthcare and community-based organizations, including Coastal Horizons, disAbility Resource Center, NCWorks Pender and New Hanover, Pender County Christian Services, Pender County Health Department, RHA Wilmington, Smart Start New Hanover, and Step Up Wilmington.

With growing awareness of the many social, economic, and environmental factors that influence health, the network is a cutting-edge initiative that will transform care delivery in North Carolina and inform policies nationwide. This platform provides a solution to a fragmented health and human services system, by connecting providers across sectors. NCCARE360 ensures accountability around services delivered, provides a “no wrong door” approach, closes the loop on every referral made, and reports on the outcomes of that connection.

NCCARE360 Now Serving Eastern NC in Pitt, Edgecombe, Martin, Bertie, Chowan, Hertford, and Beaufort Counties

August 19, 2019 – NCCARE360, the statewide coordinated care network connecting individuals to local services and resources, is now serving Pitt, Edgecombe, Martin, Bertie, Chowan, Hertford, and Beaufort counties. This third rollout has launched with healthcare and community-based organizations, including: OIC of Rocky Mount, Access East, The REACH Center, Albemarle Alliance for Children and Families, and The Wright’s Center.

NCCARE360 creates an infrastructure to connect all North Carolinians to community resources by building a collaborative network of healthcare and human services organizations with a shared technology platform that allows for a coordinated, community-oriented, person-centered approach to delivering care in North Carolina. NCCARE360 allows providers to electronically connect those with identified needs to community resources and allow for a feedback loop on the outcome of that connection. This solution ensures accountability around services delivered, provides a “no wrong door” approach, closes the loop on every referral made, and reports on the outcomes of that connection. NCCARE360 is a groundbreaking tool that has the power to transform lives and impact communities across North Carolina.

“NCCARE360 helps community-based organizations because it automates and standardizes connecting community residents to services essential to their health and well-being,” shared Caroline Doherty, Chief Development and Programs Officer for the Roanoke Chowan Community Health Center in Hertford County.

“Eagle’s Wings Food Pantry believes this new network will help many people in Beaufort County. Many of our folks just don’t know where to go for specific assistance, and this new tool makes it possible to connect those who need with those who can help,” discussed Ann-Marie Montague, Executive Director.

North Carolina, through NCCARE360, is the first state in the country to create a shared statewide technology infrastructure and coordinated community network uniting health care and human services, enabling providers to connect North Carolinians to the resources they need to be healthy, safe and well.

Ronald Gaskins, Vice-President, Population Health Management, Vidant Health remarked, “The recent expansion of NCCARE360 will be a valuable resource as we strive to bridge the communication gap between social and medical interventions that influence health outcomes. Across Eastern North Carolina, we experience higher prevalence of chronic disease, which is only magnified by the prominent role that social determinants of health have within the disease state itself. Having NCCARE360 will help facilitate efficient coordination of services, both medical and social, and that is so important when dealing with finite resources.”

As NCCARE360’s reach continues to expand, it will be available in every county in North Carolina with full statewide implementation by end of 2020. NCCARE360 has already launched in Guilford, Alamance, Rockingham, Wake, Johnston, Pitt, Edgecombe, Beaufort, Martin, Hertford, Bertie, and Chowan counties. Implementation efforts are currently underway in New Hanover, Brunswick, Pender, Vance, Granville, Franklin, Warren, Durham, Person counties.

Greensboro Housing Coalition: Partner Spotlight

Today, we are sitting down with Robbielene Lawhorn, at Greensboro Housing Coalition to discuss the organization’s achievements, experience with the network, and vision for the future.

How would you summarize Greensboro Housing Coalition’s work and your role in particular? What populations does Greensboro Housing Coalition serve?

The Greensboro Housing Coalition is an agency that is concerned with safe and affordable housing. We believe that everyone has the right to stay in a decent place that they can afford. Most days, we find ourselves acting as a triage point for people about a wide variety of housing situations. We listen to the concerns of the community and we work to provide counseling and solutions. In some instances, we address their concerns through programs that we offer and also often have to refer them out when there are circumstances that GHC may not be able to address. My role as the Homeless Prevention Team Manager is directing my team as we field phone calls and see clients face to face regarding their housing issues. Our agency serves anyone. We get calls from all over the U.S. regarding housing concerns. Most of our clients have low income and are in crisis when they walk through our doors or call us.

Currently, what’re your biggest challenges at Greensboro Housing Coalition? How do you deal with them?

The biggest challenges that we have at GHC are the scant funding resources that we can tap into for our clients for things such as emergency assistance or eviction prevention. The other huge issue is the lack of enough safe and affordable housing properties that we can direct people to. We help clients think out of the box to find monetary resources and negotiate what they might owe and/or need. Like most communities, Greensboro is under-resourced to deal with the types of crisis that vulnerable people find themselves in on a daily basis. According to recent data, Greensboro ranks #7 nationally for numbers of evictions.  

What accomplishments are your team most proud of?

The thing that I am most proud of for my team and GHC as a whole is that we rise to the occasion in times of need. When there is a disaster, such as the tornado that hit Greensboro in April of 2018, when a multi-unit apartment complex was condemned, and when properties have been flooded or there has been a fire leading to condemnations through no fault of the resident, we help to re-house people. During times of large and small scale housing emergencies, GHC is one of the first agencies, and in some cases the only agency, that is called upon to analyze what is happening, develop a plan of action to help the person/people affected and to implement that plan. We work closely with other community partners to help our clients, if and when necessary. Our GHC staff stays ready and willing to help out in any way that we can.

How has NCCARE360 impacted the way you deliver care? What has surprised you?

The use of NCCARE360 has allowed us the ability to be able to reach people who might not have ordinarily contacted us because they were unaware of our agency, through referrals from other member agencies. We have received many more referrals from healthcare partners than before. 

What was it like for your organization before working with NCCARE360? And afterward?

We have always been an agency that people referred their clients, friends, and family to for possible help. Now, we are reaching more people. We strongly appreciate the feedback loop that is built into NCCARE360 between agencies making and receiving referrals. In the past, referrals have come our way for things that we can’t or don’t do, and the clients have had to come to us to find this out. NCCARE360 has streamlined things for the consumer, saving them time so they don’t need to use resources to travel to an agency that might not be able to help them. 

What advice would you give to an organization interested in joining NCCARE360?

I would suggest that any organization join because it will give them access to potential clients who might not have ever known about them. It also provides possible resources that they could refer their own clients to. With more resources, people’s situations may be resolved quicker.

What’s next for Greensboro Housing Coalition?

GHC is working to build relationships with NC’s Prepaid Health Plans and is working to be part of Medicaid Pilots if our region is selected. We believe that there has already been a demonstrated health benefit of going upstream to address social drivers of health and we want to continue to help push conversations about the most effective ways to deliver care and serve vulnerable people.

Alamance County Children’s Dental Clinic: Partner Spotlight

Today, we are sitting down with Angie Sloate and Stacie Saunders at Alamance County Children’s Dental Clinic to discuss their achievements, experience with the network, and vision for the future.


How would you summarize Alamance County Children’s Dental Clinic’s work and your role in particular? What populations does Alamance County Children’s Dental Clinic serve?

Angie: I am a Patient Account Representative in the front office. We are a public health agency that provides dental care for the citizens of Alamance and Caswell Counties. We see children ages 0-21. We serve those with Medicaid, private insurance, and the uninsured.

Stacie: Alamance County Children’s Dental Clinic provides a dental home for children in our community. They provide services that you might find any pediatric dental office. Being a public health dental clinic, we also provide a great deal of health education and work to identify unmet dental needs in our community through various partnerships. 

Currently, what’re your biggest challenges at Alamance County Children’s Dental Clinic? How do you deal with them?

Angie: We are always trying to reach the children in our community, especially the ones that may not be able to afford dental care. We want to educate and promote preventative dental care. We are on a limited budget, therefore, we are not able to advertise our services as much as we would like. To alleviate this, we try to go out to community events and back-to-school fairs in order to promote and educate people about our clinic and the services we provide. 

Stacie: There are everyday challenges that any medical clinic deals with, like folks not being able to make their appointments. These daily challenges are sometimes indicative of more deeply rooted drivers of health like limited transportation or inability for a caregiver to leave work. Currently, the dental clinic has a planning grant that is allowing us to work with our school system partners to provide basic dental care at school. 

What accomplishments are your team most proud of?

Stacie: The Alamance County Children’s Dental Clinic is fully sustaining, meaning it receives no local tax dollars for its operation. This is almost unheard of in public health clinical care and really is a testament to our staff’s dedication to seeing patients. It also speaks to the quality of care that our patients receive, that they continue to trust us with their care. 

How has NCCARE360 impacted the way you deliver care? What has surprised you? 

Angie: With NCCARE360, we are able to reach children at a younger age. In turn, we are able to educate parents on how to take care of their children’s teeth, and possibly prevent any decay/cavities from happening at such a young age. 

Stacie: I have been surprised at the increased level of communication and referral between our Women, Infants, and Children (WIC) program and our dental clinic. Not having to make a call or leave it up to the client to make the call has really improved the referral system between WIC and Dental. 

What was it like for your organization before working with NCCARE360? And afterward?

Angie: Before NCCARE360, the WIC/Health Departments would hand out brochures to clients and they may or may not have called us to schedule an appointment. NCCARE360 allows us to reach children that we were not able to reach before by automating the referral process and contact information. 

Stacie: NCCARE360 allows our staff to make real time referrals for a patient, and within a few hours we have a feedback loop closed.

Based on your experiences so far, how do you see NCCARE360 impacting the standard of care in North Carolina?

Angie: I feel that NCCARE360 will change the way we do health care if all the community agencies participate. 

Stacie: I am most excited about what we will learn about our local resource networks as a result of NCCARE360. Being able to look at data a year from now and see where they are going, what we are still missing, and what we do well, will help us learn how to build on our strengths and fix any problems we’re having. 

What advice would you give to an organization interested in joining NCCARE360?

Angie: If you truly want to help your clients, using NCCARE360 opens up a world of opportunities for them. It helps you get to know more about each client. You can sometimes find out that when a patient needs help in one part of their life (be it a job, housing, or transportation) it may be affecting another aspect of their life (such as education, health, etc.).

Stacie: Contact the Community Success Team! They can help you identify the potential of the platform for your agency. They really do spend time with you thinking through the workflow so you can be as successful as possible and better serve your clients. 

What’s next for Alamance County Children’s Dental Clinic?

Stacie: School-based dental care. In addition to the planning grant, the dental clinic received an implementation grant to provide basic dental care in local schools. We hope that we see an impact on the dental health of those we serve and also align our goals to those of the school, like reducing absenteeism.

Guilford, Alamance, and Rockingham Launch Party

This summer, NCCARE360 officially launched in Guilford, Alamance, and Rockingham counties! To celebrate, we hosted a launch party on June 3rd for our partners and supporters.

Speakers included Anne Thomas, President/CEO, Foundation for Health Leadership & Innovation; Laura Marx, President/CEO, United Way of North Carolina; Betsey Tilson, State Health Director & the Chief Medical Officer, DHHS; Mandy Cohen, Secretary, DHHS; and Morgan Forrester, Community Engagement Manager, Unite Us.

 

The launch party also featured testimonials from two partners; Jackie Lucas, Executive Director, The Salvation Army of Greensboro and Kathy Colville; Director of Healthy Communities, Cone Health. Community partners are leading the way as we grow the network, and their support is crucial for this process. Before NCCARE360, the health and human services landscape was fragmented. As Kathy shared during the event, “This tool works because we are humans connected to each other.”

 

 

 

Secretary Cohen helped emphasize this spirit of connectivity, sharing that “we are knitting together the resources we have across this state…We’re going to teach the country how to get it done.”

Our June 3rd launch was such an inspirational afternoon, as the room was filled with optimism. Thank you to all the launch party attendees and NCCARE360 supporters! As we continue working towards nationwide coverage by 2020, this event captured the determination and drive of NCCARE360.

 

Temple: Partner Spotlight

Today, we are sitting down with Tiffany Joseph, Ministry Assistant at Temple to discuss their achievements, experience with the network, and vision for the future.


How would you summarize Temple’s work and your role in particular? What populations does Temple serve?

Temple is a church that loves everyone in our community. We offer Celebrate Recovery and homeless outreach, as well as being a church that believes in God’s word. I am Celebrate Recovery Ministry Assistant. I take all of our CR calls and am available 24 hours a day for that purpose. Additionally, I help with the ministry at the church, go to community meetings, and work one-on-one counseling women. We serve our whole community as CR is a 12 step program for any hurt, habit, or hang-up which can include drugs, alcohol, anxiety, depression, codependency. It’s literally for anything someone might have an issue with. We also serve in the women’s prison in Raleigh, NC and two men’s prisons in Harnett County.

Currently, what’re your biggest challenges at Temple? How do you deal with them?

The biggest challenge at the moment, is not enough people knowing that Temple is involved with NCCARE360. We really need to work on getting the word out for this great community service. Thus, we will be adding this information to our Friday night announcements. 

What accomplishments are your team most proud of?

As a team member at Temple, I am very proud of the fact that we are reaching out to those others often neglect, ignore, and judge. Showing love to all is very important to us.

How has NCCARE360 impacted the way you deliver care?

NCCARE360 has given us the necessary tools to help those in need find help to improve their lives – be it clothing, food, job search, etc. It is so wonderful to be able to see the process from start to finish and know that a person that came to us for help got what they needed. 

What was it like for your organization before working with NCCARE360? And afterward?

Before, it was just a conversation of trying to tell someone where they could maybe get help. Now, we have the tool to know where that help is available. It is so wonderful to have this at our fingertips.

What advice would you give to an organization interested in joining NCCARE360?

Please join and help your community to serve those in need. The more organizations we have, the more people we can help.

What’s next for Temple?

We are in the process of finding a property for a Dream Center for families in a domestic violence situation. At this center, we hope to assist those affected by having a safe place to stay, help with education needs, clothing needs, and eventually helping them find a new home.

Anything else you would like to add?

I am so excited for what the future holds for Temple, our community, and NCCARE360. We can achieve so much when we all work together for the greater good.

NCWorks: Partner Spotlight

Today, we are sitting down with Renee Hinton, Assistant Center Manager at NCWorks Career Centers Clayton to discuss their achievements, experience with the network, and vision for the future.


How would you summarize NCWorks’s work and your role in particular? What populations does NCWorks serve?

NCWorks Career Centers assist people with training and employment and everything in between. We provide one on one assistance with resume writing, career counseling and exploration, job search, training, interviewing, etc. We connect people with jobs and connect talent to employers’ job openings. We can assist employers with labor market data, staffing challenges, employee/applicant skills gaps, and outplacement services. Our center serves the adult population, while the youth population is served by NCWorks NextGen Centers. Our services are free to applicants and employers. At NCWorks Career Center Clayton, my role is to assist in the center’s operations, oversee staff, assist customers, coordinate programs/activities, work with partner agencies, etc.

Currently, what are your biggest challenges at NCWorks? How do you deal with them?

Our biggest challenge currently is matching the skills of people with the requirements of the positions available. However, we have a variety of programs to bridge this gap. We provide opportunities for educational training, work experience, on the job training, and apprenticeships. We also offer free workshops on a variety of subjects and to a variety of populations. We also face marketing challenges, as most of our marketing is through word of mouth. NCWorks – Clayton has only been in the area for a few years, but we are always happy to try to spread the word of who we are and what we do.

What accomplishments are your team most proud of?

Our awesome staff serves an average of 600 people a month in our office. In addition, we have customers who connect with us through our website or call center. We are very proud of our work and that it helps citizens to not only be productive contributing members of our community, but also helps them to provide for their families. As for the employers, we are happy we are able to assist them to grow and stay in North Carolina.

How has NCCARE360 impacted the way you deliver care? What has surprised you?

Before NCCARE360 we used a notebook of resources which we had to update regularly and became cumbersome and time consuming. NCCARE360 provides one place to go for resources. We are able to make multiple referrals in a short amount of time knowing the information is current, and the receiving agency will respond. I believe the NCCARE360 is a great resource, and the surprising part is how easy it is to use. It is very user friendly in my opinion.

What was it like for your organization before working with NCCARE360? And afterward?

Before NCCARE360, we could make a referral and not necessarily know if the person received help unless we followed up and spoke with the agency or client. Now we are able to follow the progression in NCCARE360. 

Based on your experiences so far, how do you see NCCARE360 impacting the standard of care in North Carolina?

Once the system is expanded to cover North Carolina, I can see us making referrals to anywhere in the state for our clients! Many citizens do not stay in one place and may move around. This will allow us to be able to connect people with resources anywhere in the state easily and efficiently. It allows our state to be connected on a resource level that has not been available before. There are many resources available in NCCARE360, and it is a database that can expand as time goes. Allowing all of North Carolina to be connected in assisting our citizens with many needs across our fine state. 

What advice would you give to an organization interested in joining NCCARE360?

Join! It is such an easy way to connect your clients with resources across the state. It also allows providers across the state to connect with you — a win-win situation for providers and clients. Send referrals through NCCARE360, and we will be glad to help your customers as well.

NCCARE360 Continues to Expand in North Carolina with Rollouts in Johnston and Wake Counties

NCCARE360, the statewide coordinated care network connecting individuals to local services and resources, has officially launched in Johnston and Wake counties. This second rollout has launched with healthcare and community-based organizations, including WakeMed, NCWORKS Career Center, Johnston County Health Department, Catholic Parish Outreach, Duke Raleigh Hospital, Vocational Rehab of Raleigh & Johnston County, and NeighborHealth.

With growing awareness of the many social, economic, and environmental factors that influence health, it is critical to integrate services along the continuum. Today, however, the system to navigate these services is fragmented. When providers make referrals to other organizations, the process is often manual, time-consuming, and does not allow them to understand what happened to their client once the referral was sent. NCCARE360 is a collaborative solution to this problem providing a coordinated, community-oriented, person-centered approach to delivering care in North Carolina.

“We made a referral at 10pm and it was accepted at 8am the next morning, I was so excited to tell our client that she would get the help she needed,” says Tiffany Joseph, Celebrate Recovery Ministry Assistant at Temple. “I’ve been sharing what NCCARE360 is with everyone, family and friends because this is an amazing resource. This is really going to help so many people. I hope we get more community members to get onboard because I think it’s phenomenal.”

Through NCCARE360, North Carolinians are connected to behavioral health, clinical, and social service partners, dedicated to working together to improve health. NCCARE360 includes a call center with dedicated navigators and a shared technology platform that enables providers to send and receive secure electronic referrals, communicate in real-time, securely share client information, and collect data on factors impacting outcomes and costs.

“As the evolving world of population health continues to move outside of our medical silos and into the communities where our patients live, the NCCARE360 platform will be an essential tool that will facilitate the communication and coordination we need as a community to improve the health of our residents,” says Dr. Brian Klausner M.D from WakeMed.

Health Happens Locally – Welcome To NCCARE360 By Anne Thomas

Together, we are changing the trajectory of health for all North Carolinians.

There is growing recognition that better coordination and investment in the non-medical drivers of health, like access to healthy food, safe and affordable housing and well-paying jobs, can improve health and decrease health care costs. Through NCCARE360’s groundbreaking, statewide coordinated network, health care and human services organizations are now united through a shared technology platform. NCCARE360 is a coordinated, community-oriented, person-centered approach for delivering care in North Carolina, and sets an example for the entire nation. In fact, this transformation is already underway in our state and rapidly expanding to support every person in North Carolina.

We have launched in 12 counties, with 9 counties in the implementation process. Over 1000 organizations have engaged in the socialization process and we look forward to working closely together.

NCCARE360 provides the infrastructure to connect all North Carolinians to community resources and has the potential to touch the lives and improve the health of all North Carolinians, including the commercially insured, Medicare, Medicaid, uninsured, military and veteran populations. NCCARE360 helps build a system of health that is focused on the person and helps them access the services and resources they need to be healthy. Together we’ve invested in both existing and new infrastructure to enable different types of organizations to connect people to health and social services in a way that makes the system more efficient. “To build a healthier North Carolina, we must build a system of health that unites health care and human services,” said DHHS Secretary Mandy Cohen. “NCCARE360 is a scalable, coordinated solution that makes it easier for providers, insurers and community organizations to connect people with the resources they need to be healthy and delivers value for all.”

Through NCCARE360, community partners have access to a robust statewide resource data repository that will include a call center with dedicated navigators and a shared technology platform that enables health care and human service providers to send and receive secure electronic referrals, communicate in real-time, securely share client information, and track outcomes. This solution ensures accountability around services delivered, provides a “no wrong door” approach, and closes the loop on every referral made.

“NCCARE360 will help us elevate the care we provide our patients,” says Kathy Colville, healthy communities director, Cone Health. “High quality medical care combined with healthy housing and nutritious food are a prescription for good health. NCCARE360 helps us make those connections.”

“We recently received a referral indicating a lady’s home was about to be condemned. Through NCCARE360 we connected the client to another agency and her issue was tracked and resolved within 24 hours,” Says Jackie Lucas, Executive Director, Salvation Army of Greensboro Center of Hope. “NCCARE360 eliminates unnecessary barriers for the client and the provider. Now the provider can see the services the client was receiving as they deliver care for the same person.”

NCCARE360 will be available in every county in North Carolina with full statewide implementation by end of 2020. On the ground implementation efforts are currently underway in Wake and Johnston counties followed by Pitt, Edgecombe, Beaufort, Martin, Hertford, Bertie, and Chowan counties.
